The Department of Health Jackson County (JCHD) is able to assist you with obtaining immediate Medicaid IF you are qualified. This applies only to pregnant women. To determine if you are eligible, you must come to JCHD with photo identification and complete an application on site.
Your application will be processed and you will obtain immediate notification of program acceptance or denial.
You may print the form from the link below or we will provide you with one upon coming to the Health Department. Appointments are preferred but not required.
Simplified Eligibility for Pregnant Women (SEPW): A simplified "full coverage" for pregnant women only. Please complete the one page application linked below. Print the application and bring it with you to the Health Department.
