COMMUNITY HEALTH ASSESSMENT

During April and May 2013, the Jackson County Community Health Partnership completed a comprehensive county-wide Community Health Assessment using a nationally recognized approach called Mobilizing for Action through Planning and Partnerships (MAPP). The Partnership then used the data collected in the County Health Assessment to develop a Community Health Improvement Plan.

Championed by the Florida Department of Health in Jackson County, the Jackson County Community Health Partnership is comprised of a diverse group of health, governmental, faith-based, social service, business, educational, civic, and community-based organization leaders that make up the local systems.  The Community Health Partnership's mission is to promote, protect and improve the health of all people in Jackson County.

The 2013 Community Health Assessment identified a number of strategic health objectives that provide opportunities for improvement in Jackson County.  Following analysis of community feedback and statistical health data that was collected for the Assessment, the Community Health Partnership developed an overarching list of community health objectives where they could have the greatest impact.
These objectives include:

          Reduce communicable disease and infectious rate in Jackson County
          Increase availability of healthcare servicesaccess in Jackson County
          Incorporate certified vocational training in the Jackson County High School curriculum

Moving forward, progress will be monitored and evaluated, based on the measures and performance targets established.
