Jackson County CHA 

Jackson County Health Department, Jackson Hospital and the Health Planning Council of Northeast Florida worked in collaboration to produce the 2019 Jackson County Community Health Assessment (CHA).  This assessment shares significant community health indicators identified through modified Mobilizing for Action through Planning and Partnerships (MAPP) strategic planning.
