Jackson County Community Health Assessment

The Florida Department of Health in Jackson County and the Jackson Hospital worked in collaboration to produce the 2022 Jackson County Community Health Assessment (CHA). This assessment shares significant community health indicators identified through modified Mobilizing for Action through Planning and Partnerships (MAPP) strategic planning. 

Jackson County Community Health Improvement Plan

A Community Health Improvement Plan (CHIP) is a long-term, systematic effort to address public health problems based on the results of the CHA activities and the Community Health Improvement process. That Jackson County CHIP group has brought stakeholders together to better understand the needs of Jackson County. It is used by health, educational, and human services agencies as well as other governmental agencies, in collaboration with community partners, to set priorities and coordinate and target resources to improve the health of the communities they serve. 

Social Services and Outreach

The Social Service Division of the Florida Department of Health in Jackson County is commonly known as the outreach section of the facility, that facilitates the Community Health Initiatives and Support. 

It is our goal to provide the needed support to our county residents and visitors that assure good health outcomes for our clients.
